  1. Have you, or anyone on site, recently travelled outside of Canada (including the US) in the last 14 days?
  2. Do you or anyone at the premises have a flu-like illness (fever, cough, sore throat, muscle aches, and fatigue), or been sent home for displaying these symptoms?
  3. Have you or anyone at the premises had close contact with a person known to have COVID-19, is otherwise displaying flu-like symptoms or is suspected of having COVID-19?

Further, we ask that the premise be limited to our inspector(s) for the purpose of their inspection and activation.
